Where to Submit Your Request
Please submit your completed Employment Verification Request Form to the employee’s HR Partner or Timekeeper by email, fax, or mail. If you are unsure of the HR Partner’s or Timekeeper’s contact information, you may obtain it by contacting the Office of School Support Services at (718) 707-4300. You will be required to provide the employee’s name and Social Security number.
